Is square footage heated space or does it include the garage?

Heated space. Garages, porches and decks are drawn and dimensioned but are not counted in the figure, which is the convention and the one that matters for a build estimate.

Can the plan be modified?

Yes. The PDF is yours and most buyers take it to a local draftsman or engineer for the stamp and any changes their lot needs.
